* All parents/guardians must submit a waiver of release, registration form, and educational background form by June 1st in order to begin sessions on Monday June 8th.

* As soon as paper work is processed a time will be arranged. The times are scheduled on a first come first served basis. So get your paperwork in early to be at the front of the line.

*Please contact me by e-mail or phone to arrange delivery of paperwork (paperwork can either be sent via e-mail, fax, or snail mail).
